Just fill out the application attached and email it back to us with the proper credentials. Upon approval you will receive your MUA discount code to use at checkout. • Include a copy of your photo ID.

B) Include two of the following credentials:

• Magazine Tear sheet with your name credit*

• Agency comp card

• Makeup Artist business card

• Union card*

• Crew/call list from a production company with letterhead*

• Professional license*
